Fıtık Nedir?
Fıtık, iç organlardan birinin vücudun zayıf bir bölgesinden dışarıya doğru çıkmasıdır. Sıklıkla karın bölgesinde, karın bölgesinde ise en sık kasıkta görülür. Kasık fıtığının tıbbi adı ”inguinal herni”dir.
Karın bölgesinde en sık görülen ikinci fıtık ise tıbbi olarak umblikal herni olarak adlandırılan göbek fıtığıdır.
Karın bölgesinde en çok kasık ve göbek fıtığı görülse de karnın başka zayıf bölgelerinde de fıtık oluşabilir. Fıtık kesesinin içinde en sık bağırsaklar görülür ancak nadiren de olsa diğer organlara rastlanabilir.
Doğuştan fıtık olunabileceği gibi yaşam şekline bağlı olarak oluşan fıtık çeşitleri de vardır. Şişmanlık, kabızlık, ağır yük kaldırma , şiddetli öksürük ve karın kaslarını zorlayıcı hareketler fıtık oluşumuna yol açabilir. Karın kasları zayıf olan bebeklerde sürekli ağlama ve kabızlık, fıtık oluşumunda başlıca faktörlerdir. Tüm bunların yanı sıra geçirilmiş karın ameliyatlarından sonra ameliyat bölgesinde de fıtık oluşabilir.
Fıtık belirtileri nelerdir?
Başlıca fıtık belirtileri ağrı ve şişliktir. Karnın zayıf bir bölgesinde bağırsaklar fıtıklaşmışsa üzerine basıldığında gazın hareketi hissedilebilir. Eğer fıtık sıkışırsa yada boğulursa bulantı, kusma ve çok şiddetli ağrı ortaya çıkar. Bazı durumlarda fıtık hissedilmez ancak ıkınma ve öksürme ile ortaya çıkabilir.
Kasıkta fıtık
Kasık fıtıkları tüm karın fıtıklarının %90’ını oluşturur. Erkeklerde daha sık görülür. Göbek fıtığı ise kadınlarda daha yaygındır. Kasık fıtığı bebeklikten yaşlılığa kadar her yaşta görülebilir. Kasık fıtığında şişlik kasıktadır. Ayaktayken ve hareket halinde şişlik belirginken yatarken düzelebilir. Şişlik dışındaki diğer belirtiler ağrı ve kabızlıktır. Şişlik görülmeksizin sadece kasıkta ağrı ile karakterize fıtık türü de vardır. Buna nokta fıtığı denir. Daha ziyade sporcular ve atletik yapılı kişilerde görülür. Nokta fıtığının tanısı ultrasonografi ile konur.
Bebeklerde fıtık, bebeğin sürekli ağlaması ile kendini gösterir. Muayenede kasık bölgesindeki şişlik tanı koydurucudur. Bazen şişlik testis torbalarına kadar uzayabilir. Bebeklerde bu tabloya sıklıkla kabızlık eşlik eder.
Fıtık tedavisi nasıl yapılır?
Zamanla düzelme ihtimali olduğundan bebeklerde görülen göbek fıtığının tedavisi çocuk 4 yaşına gelinceye kadar bekletilebilir. Kasık fıtıkları ise hangi yaşta görülürse görülsün cerrahi olarak onarılmalıdır. Sürekli yerine girip çıkan fıtık; çok şiddetli ağrı, bulantı ve kusmaya sebep olabilir. Şişlik çok serttir ve fıtık kesesi geriye itilemez. Bu durum acil cerrahi müdahale gerektirir. Halk arasında sıklıkla kullanılan fıtık bağı ve korsenin tıbbi tedavide yeri yoktur.
